These Terms of Service (“Terms”) govern your use of the website located at www.leanlifefunctionalmedicine.com and your purchase or use of services, programs, consultations, educational materials, and other offerings provided by Lean-Life Functional Medicine LLC (“LLFM,” “we,” “us,” or “our”).
By using this website, purchasing a service, scheduling a consultation, enrolling in a program, or otherwise using our services, you agree to these Terms.

Lean-Life Functional Medicine LLC is a Wyoming limited liability company providing virtual wellness education, health coaching, nutrition and lifestyle education, laboratory education, and related wellness services.

Dr. Timothy Hotz, D.C., is a member of Lean-Life Functional Medicine LLC and may provide educational and wellness services through LLFM. His professional credentials and licensure do not change the nature of services described in these Terms.
NATURE OF OUR SERVICES
LLFM provides educational and wellness-oriented services designed to help clients better understand factors that may affect their health and wellness.
Depending upon the program or service purchased, services may include:
• Wellness and lifestyle education;
• Nutrition and food-related education;
• Educational discussion of laboratory findings;
• Wellness coaching;
• Information regarding vitamins, minerals, nutritional supplements, and lifestyle strategies;
• Education regarding sleep, physical activity, stress management, environmental exposures, and other wellness factors;
• Educational materials, videos, courses, webinars, consultations, and coaching sessions; and
• Other wellness-related services described at the time of purchase.
The exact services included in a particular program are determined by the program description and, when applicable, the client's enrollment agreement.
EDUCATIONAL SERVICES — NOT MEDICAL TREATMENT
LLFM's virtual wellness and educational programs are not intended to replace medical care from a physician or other appropriately licensed healthcare professional.
Unless expressly established through a separate written agreement for professional healthcare services, participation in an LLFM educational or wellness program does not establish a physician-patient or chiropractic patient relationship.
Information provided through LLFM's website, programs, consultations, coaching, educational materials, videos, webinars, emails, or other communications should not be considered a substitute for individualized medical diagnosis or treatment.
LLFM's online wellness programs are not intended to diagnose disease, prescribe prescription medications, provide emergency medical services, or direct clients to discontinue prescribed medications.
Clients should maintain an appropriate relationship with their physician or other licensed healthcare professionals for diagnosis, medical treatment, prescription medication management, and other healthcare needs.
Never disregard professional medical advice or delay seeking medical care because of information received through LLFM.
If you believe you are experiencing a medical emergency, call 911 or seek appropriate emergency medical care immediately.
LABORATORY TESTING AND EDUCATIONAL REVIEW
Certain LLFM programs may include or involve laboratory testing.
When laboratory testing is used, specimen collection and laboratory analysis may be performed by independent laboratories, blood-draw facilities, specimen-collection locations, or other third-party providers.
LLFM does not represent that it personally performs laboratory analysis or blood-draw services when those services are provided by independent third parties.
Laboratory findings may be reviewed with clients for educational and wellness purposes to help clients better understand information relevant to nutrition, lifestyle, and wellness planning.
Clients should discuss laboratory findings requiring medical diagnosis, medical treatment, prescription medication changes, or other clinical medical decisions with an appropriately licensed healthcare professional.
NUTRITION, SUPPLEMENTS, AND LIFESTYLE INFORMATION
LLFM may provide educational information concerning foods, nutrition, vitamins, minerals, nutritional supplements, physical activity, sleep, stress management, environmental factors, and other lifestyle considerations.
Individual responses to foods, supplements, lifestyle changes, and wellness strategies vary.
Nutritional supplements and dietary or lifestyle changes may not be appropriate for every individual and may interact with medications, medical conditions, allergies, or other circumstances.
You are responsible for informing your physician, pharmacist, or other appropriate healthcare professional about supplements and significant dietary or lifestyle changes when appropriate.
Do not stop or alter prescription medication based solely upon information provided through an LLFM educational or wellness program.
NO GUARANTEE OF RESULTS
LLFM does not guarantee any particular health, wellness, laboratory, weight, blood sugar, medication, financial, or other outcome.
Testimonials, examples, case histories, or statements regarding experiences of other clients represent individual experiences and should not be interpreted as a promise or guarantee that you will achieve the same or similar results.
Your results may vary based upon numerous factors, including your individual circumstances, participation, consistency, lifestyle, health status, and factors outside LLFM's control.
CONSULTATIONS AND APPOINTMENTS
LLFM may charge a fee for consultations or other appointments.
The price and nature of a consultation will be disclosed before purchase.
Scheduling an appointment does not guarantee acceptance into any LLFM program.
LLFM may determine that its programs are not appropriate for a prospective client and may recommend that the individual seek assistance from another provider or professional.
Cancellation, rescheduling, missed-appointment, and refund provisions are governed by LLFM's Refund, Cancellation & Rescheduling Policy and, where applicable, the terms of an individual enrollment agreement.
PROGRAM ENROLLMENT AND ADDITIONAL AGREEMENTS
Certain LLFM programs may require clients to review and sign a separate enrollment agreement.
That agreement may contain additional provisions concerning:
• Program duration;
• Services and materials included;
• Laboratory testing;
• Coaching;
• Supplements;
• Payment schedules;
• Financing;
• Cancellations;
• Refunds; and
• Other program-specific terms.
If a program-specific written enrollment agreement contains terms that differ from these general website Terms regarding that particular program, the program-specific agreement will control to the extent of that difference.
PAYMENTS AND PAYMENT PROCESSING
Prices are displayed or communicated before purchase.
Payments may be processed through third-party payment processors, including Stripe or other payment providers used by LLFM.
By submitting payment information, you represent that you are authorized to use the selected payment method and authorize the applicable charges associated with your purchase.
LLFM does not guarantee that any particular payment method, financing option, or payment plan will always be available.
If you enroll in an installment or payment plan, you remain responsible for payments according to the agreement applicable to that purchase unless otherwise provided by law or the applicable written agreement.
REFUNDS, CANCELLATIONS, AND RESCHEDULING
Refunds, cancellations, appointment rescheduling, and missed appointments are governed by LLFM's Refund, Cancellation & Rescheduling Policy and any program-specific enrollment agreement.
Because LLFM may incur costs for laboratory testing, specimen collection, supplements, shipping, third-party services, digital materials, or other services shortly after enrollment, certain costs may become non-refundable once ordered, purchased, shipped, accessed, performed, or otherwise incurred.
Clients should review the applicable refund and cancellation terms before completing a purchase.
THIRD-PARTY SERVICES
LLFM may use or refer clients to independent third parties, including laboratories, specimen-collection facilities, supplement suppliers, scheduling platforms, payment processors, financing providers, technology platforms, and other service providers.
Unless specifically stated otherwise, these third parties are independent from LLFM.
Their products and services may be subject to their own terms, privacy practices, pricing, policies, and availability.
LLFM is not responsible for acts, omissions, delays, service interruptions, or other matters outside LLFM's reasonable control involving independent third-party providers.
CLIENT RESPONSIBILITIES
By participating in LLFM services, you agree to provide information that is accurate and complete to the best of your knowledge.
You are responsible for your own decisions regarding whether and how to use educational information provided by LLFM.
You are also responsible for seeking appropriate licensed medical care when needed and for communicating with your healthcare professionals regarding matters requiring medical diagnosis, treatment, prescription medication management, or emergency care.
WEBSITE AND EDUCATIONAL CONTENT
All website content and educational materials are provided for informational and educational purposes.
LLFM attempts to provide useful and accurate information but does not warrant that all website or educational content will always be complete, current, error-free, or appropriate for every individual.
Information concerning health, nutrition, wellness, laboratories, supplements, technology, and related subjects may change over time.
INTELLECTUAL PROPERTY
Unless otherwise stated, the content provided by LLFM—including written materials, videos, presentations, graphics, program materials, training materials, worksheets, educational systems, branding, and other original content—is owned by or licensed to Lean-Life Functional Medicine LLC and is protected by applicable intellectual property laws.
Purchasing or accessing LLFM services gives you a limited, personal, non-exclusive, non-transferable right to use the materials for your own personal purposes.

INDEMNIFICATION
